A web-based system that was designed exclusively for the barangay health centers within Manila area. This system aims to solve the problems encountered in manual operation of medical records. The system focused in the management of patient records (add, edit, delete and update), monitoring of patient medical records, generate reports (annually, quarterly, monthly and custom period) and contact support module for system maintenance. The system was developed using Sublime Text, MySQL, and PHP. The system was evaluated as to its functional suitability, usability and portability and gained a frequency mean rating of 3.93 with a descriptive rating of *Very Acceptable"--Author's Abstract
