This study aims to develop a Beneficiary Information Management System to improve the
recording of medical, burial, and financial assistance requests at the District Office of
Congressman, 3rd District, Caloocan City. The system's main features, based on the
objectives of the study, include dashboards for the system administrator, office head, and
chief of staff, a beneficiary application form, generates analytical reports, user account
management, a search function for beneficiary records, editing and updating beneficiary
information, enabling or disabling services, and sending SMS notifications. The system
was created using PHP, MySQL, JavaScript, CSS, Bootstrap, HTML, Node.js, XAMPP,
Sublime Text 3, and SIM 800C GSM-USB. It was tested for functional suitability using
the ISO 25010 quality model, and it showed high acceptability, with a score grand weighted
mean of 3.8.
